Once again you got me thinking (ouch!)!
Here's sketch of another case I keep misprescribing for; could this be Med?
(It's not in most of the rubrics I used, but-) (Or other thoughts?)

75-ish man, silver-haired, very distinguished in dress and manner; bearing
sort of like a (usually) benevolent dictator -- when he speaks, people hop!
And when he gets annoyed, the benevolence can disappear in a wink (can be
truly embarrassing toward e.g. a waitress that offends him, altho *usually*
he would be kind and genial, perhaps even overly so).

For years I assumed he was a textbook nux-v (this is a relative that I
itched to treat, and was finally given the chance, argh), but nux-v in
varying potencies has done nothing at all, not a whit of a change. Likewise
Sulphur; platina did produce some softening, but only brief. Since then
I've been stymied.

Other rubrics I used included "affectation", as he can appear *so* dramatic,
almost as tho he is on stage! And he does love to be "on stage" -- one of
his recreations is a playreading group, where he always (!) has the (or a)
leading role. And if he is in a conversation where activity shifts such
that he is not involved enough, he will leave -- no patience with listening
while others converse!

On the other hand, he is intensely moral, active in his church (I think this
reflects his upbringing; I do not see it as excessive, but do see it as
possibly significant). He has many friends, and is close to his family
(including farflung siblings). Despite the arrogance he is also warmhearted
and caring -- so long as no one offends!

Ah yes, the chief complaint: Hurried!!! *Never* in the form of pointless
activity (so I ruled out sul-ac?) -- when working on something, or in
conversation, he is very calm, focused, and on-task. But his wife (who
begs me to find a remedy!) talks about how *irritable* he is, and
increasingly as he gets older (yeah, I've seen that!), and how he is "always
in a hurry" -- when they are getting ready to go out, he will be literally
standing in the doorway tapping his foot, making remarks like, "Do you think
we can go sometime this year?"

Other chief complaint is BHP (benign prostate enlargement), gets up several
times in the night to urinate.

In interview I asked whether he had always been ambitious. From his life
history I assumed he would say "yes", as I know he had many ambitious
undertakings in his life journey from a Kansas farm to the Chicago corporate
world, but he took a thoughtful pause and said, "I don't think I've ever
really been ambitious." Me, in surprise: "Oh, just sort of chronically
driven?" And he replied "Yes, I guess so."

He has been very successful, is surrounded by beautiful things and takes
many exotic trips (he is retired now), and he clearly loves all of that, but
it sounds as tho that was a pleasant side-effect, not necessarily the goal
of his strivings.

He tries to take things (himself included) with a good humor, but there are
times when the air around this guy just about *crackles* with barely
suppressed tension; he is *such* a "live wire"!!!

I asked other relatives re his childhood, and it seems that the major traits
he shows today were there from childhood on -- the intensity, irritability,
"drivenness" (which I would have taken for ambition), love of family, "a
habit of success".

I have been seeing the dominant miasm as Psora (the arrogance, and some
sensation that his striving is/was an attempt to fill some inner void, tho
that is nothing but impression of mine). But I am not so confident about
miasms.

Now I will make myself feel completely incompetent by asking whether Puls
could present this way??? When I rep his strong food prefs and a couple of
distinctive sxs, Puls leaps to the top. I'm aware that Puls does not always
"look like Puls", esp. in a man...

Also consider Baryta carb 200 (enlarged with impotency and Sabal serr. 1x (acute retention).
Sars 30 has less distressing symptoms than Cantharis 200, no burning, there is stramguary and many painful cramps in the bladder, urine passes in a thin feeble stream, or in drops only without pain or urging.
Cantharis 200 - burining urine and frequent urge; violent pain in bladder with tenesmus; a stitching pain occurs in neck of bladder or at its forepart when attempting to urinate; on frequent urging to urinate, can pass only a few drops; the urge is more when standing and it further increases when walking, feels better only when sitting; may be found with affections of piles

Here is an excerpt from Scholten. I know his work is not universally
accepted but it may be some food for thought on this gentleman's case as you
are stuck.

Summit Centre
This is the summit of the cycle, where everything has come to fruition.
They have made it, they have achieved what they wanted to, they have reached
the top in their career, they are the first, the best.
They are the centre of attention in all that happens. They like to be
centred and they make sure that everything remains centred around the main
theme. They are fully concentrated on their task.
Success Glitter
Their success makes them shine and glitter. They have won, they have become
first. They can be very exuberant in their happiness, almost ecstatic.

This does seem like a close description based on the info you provided.

Thanks Russell, this is *lovely*!!! It does indeed sound just like him
(still chuckling)! Okay, I will look at these some more. Plat already
