Help w/ chronic torticollis, please

Patient is 67 year old male who has a torticollis, (contracture of the left sternocleidomastoid muscle) pulling his head to the right. This began in Sept. 200l, following neck surgery in June 200l for what was dx'd as a bulging disc in his neck. His syptoms prior to surgery and leading to surgery was a numbness in thumb and lst finger tip of left hand. He can't feel anything in them still but they are very sensitive to heat and cold. (surgery did not help this numbness or sensitivity). Lately these symptoms have moved to the right lst finger and thumb. He has also had a contracture of the left 4th digit (used cortizone shot) and then went to the right. He has had botox injections in the scm muscle in May, Sept, Nov. of 2002. This helps temporarily. He is wanting to have another one next week, feeling that he has to get back to work. He is a truck driver.
Health history include blisters on the bottom of feet. Has erysipelas as a 1 year old. Appendectomy age 39. Occasionally the incision from that will break open and seep. At age 62 (1997)he had lumbar disc surgery. Before surgery he had pain standing, could not stand longer than 3 min. Sitting was fine. Pain would go from low back to knee. He does complain of frequent urination daytime only. Is on the warm side, likes to be with others or alone as well., Is on the thirsty side. Has a little claustrophobia and does not like public speaking. Nothing else too outstanding. He does have a problem with premature ejaculation. Anyone have experience with someone who has had botox injections? Presently, I have him on caust. LM l and not seeing success. Thanks for any help.
