a 50 year old man who has been smoking all his life has had this symptom on
first smoke of the day. He smokes about a pack a day.
Anyone have any idea what this could mean. His blood sugar is fine. Doesn't
happen the rest of the day.

could it be something to do with vasoconstriction affecting blood supply to the brain.
But I don't have any idea why it would only be the first one of the day.

It's called a cigarette high and this is why smokers smoke. It happens in the morning because the smoker has just had an 7-8 hour break from nicotine. It's a very normal reaction.
