I'd be happy to fill in your survey, but it does contain some problems.
(1) You ask for the child's date of birth; then you ask for the child's age! Do you mean present age, because your software can't subtract, or the age at which the child was not vaccinated, or the child's age at some other time?
(2) The question about breastfeeding provides mostly ambiguous answers. Does a child who was breastfed for three weeks go under "up to 4 weeks", or under "No"?
(3) Asking "Does your child suffer" from "illnesses" such as "Crybaby" or "Epilepsy and other seizures" obviously raises category problems. "Crybaby" is not generally recognised as an illness, is it? And do you mean "Epilepsy or other seizures", or only both?
(4) As reasons for not vaccinating a child, you provide a few good easy choices, but you forget to tell the survey participant how to select more than one at a time.
Is it possible to fix this up?
