I'd be happy to fill in your survey, but it does contain some problems.
(1) You ask for the child's date of birth; then you ask for the child's age! Do you mean present age, because your software can't subtract, or the age at which the child was not vaccinated, or the child's age at some other time?
(2) The question about breastfeeding provides mostly ambiguous answers. Does a child who was breastfed for three weeks go under "up to 4 weeks", or under "No"?
(3) Asking "Does your child suffer" from "illnesses" such as "Crybaby" or "Epilepsy and other seizures" obviously raises category problems. "Crybaby" is not generally recognised as an illness, is it? And do you mean "Epilepsy or other seizures", or only both?
(4) As reasons for not vaccinating a child, you provide a few good easy choices, but you forget to tell the survey participant how to select more than one at a time.
Is it possible to fix this up?

To make the problem with (2) clearer: a child who was breastfed to the age of 5 weeks was definitely breastfed "up to" the age of 4 weeks but not "up to" the age of 3 months. If you interpret "up to" as meaning "up to at least", then you'll pick the 4-week option for that child.

But if you interpret "up to" as meaning "no more than", then you'll pick the 3-month option.

Of course, it's no use your telling me which interpretation was the only one that occurred to you. The point is that each survey participant may interpret it either way.

I hope that that makes that problem a little more obvious than it may have been.

anti-vaccine webinars.
Sheri Tenpenny is a DO who is anti-vaccinations, too.
I am not sure what Joseph Mercola does in his practice, but he could be contacted for info.
I think these people would provide you with large numbers to work with.
And of course the Amish people who don't vaccinate.
I don't know what the stats are with Jehovah's Witnessess (I think), or Seventh Day Adventists who also
don't do medicine.

I couldn't see the multiple-selection hint till I looked for it and rolled the mouse over the "i" symbol. I'd be inclined to place the hint where it's most obvious: just under the question (and not hidden). The hint is also inaccurate for Macintosh users; the modifier key for them to use is not Strg or Ctrl but Command (Apple). (Macintoshes have a control key as well as a command key.)

I'd also get rid altogether of the question on age. Its only value is in checking what the survey respondent means by the date of birth. And that is necessary only because the format of the date is not specified. If, next to "Date of birth", you say "(yyyymmdd), then your problem is solved, and the annoyance of being asked to answer the same question again becomes unnecessary.

For the sake of engendering cooperation and trust, it'd be worth also stating what use you'd like to make of the e-mail address.

If it's important to retain the "Crybaby" option, I'd rephrase it, perhaps as "Persistent crying". Many items on that list, though, fall outside the scope of what would normally be regarded as illnesses; a better word in the question might be "symptoms".

There's a larger overall difficulty with the questionnaire, and that is the matter of what its subject is. Is it a questionnaire about the history of a single child, or about an entire family? The trouble is that it can't satisfactorily be used as either. Imagine a parent of three children -- one vaccinated, two unvaccinated -- filling in the questionnaire. She may attempt to answer the questions in relation to all three children, but it's impossible to do so. Apparently she should fill in the entire questionnaire three times altogether, once for each child; yet there is no suggestion that she should do so. The design needs to take into account such family circumstances, which will be common.
