You never want to use high C's in water except in minor acutes or first aid and only with someone with a strong vital force

Let it run certainly is not a quality way of practicing, especially in water.........yikes.
If you are confident of dosing and give good instructions, most people are fine. I have never had a person not be able to understand and use it.
