She is very "straight", law abiding, principles, that sort of thing.
She has done very well on Caust in the past, but now case seems to be
stuck. Has also developed irregular heartbeat, cardiac tests are OK,
sense is that it's stress-related.

Any ideas?

When you say she has done well on Caust - how far have you gone with it?
"Never desert a remedy that has done well." Kent.
So you may have to go high in potency.
If you have and it is having no effect, you can start low again (30C) and see what happens.
If nothing happens, then reconsider the case and use

It would seem a rational conclusion that for one who has been taught these things to react by being abiding in this way however there could be many other expressions of the discontent arising from this.

So is what needs to be cured the fact that she is abiding, obedient - in which case you might be looking at the 'servile' and related rubrics - or is it more a case of the fear of being punished that needs removing, that needs curing. One came first the other came later so to speak.

Adamas has a lot to do with blame, punishment, shame, lack of self worth (all those sort of emotional sx you might expect from being controlled in this way). They dream of their parents, become indifferent finally when they cannot expand their life ambitions. The anxieties and other felt emotions are internalised, not expressed, and can go to the heart region and an irregular heartbeat is a simple sx of this.

But there must be many rx that would fit what you describe. As always be guided by the rest of the case.

Thanks all for the input. This px (now early 30s) came originally for
psoriasis. Was greatly helped with Staph, then we were stuck for a
while. I came to the Caust on symptom similarity and also after
getting input from her family and friends which emphasized her
"crusading" qualities, fighting for others' rights etc. The Staph
related to uppermost M/E when she came, and the Caust to a deeper
level once the upper level and associated physical symptoms had eased.
The deeper level where the Caust came into play involved the return
of old symtoms (dermatitis on hands esp.) that had been suppressed
with Cortizone earlier in life. That time of life was also when the
crusading part of her personality had started emerging. The Caust
helped for around half a year, I moved her through different potencies
but I could see she'd moved into a different phase, that part of the
m/e has changed and the physical symptoms have improved greatly. Now
she's in the m/e somewhere "older", where she always had to placate a
parent, always felt she'd done wrong and was about to be punished -
although she was a very well-behaved kid. She's in the place of being
fully aware she wants to come into herself, lead her own life fully,
and I think that in itself, involving coming to terms with an
emotional separation from this parent, is part of the strain.
