Without knowing more, it is hard to judge, but is this a teenager, and is there a 
possibility/potential of self-harming ?  I ask because of the following 'very loose' 
suggested rubrics, but when someone fears hurting someone it is often a compensation of 
their desire to attack because of suppressed anger/violence.  Fear of responsibility can 
also be about a fear of growing up to maturity, and the somatisation of the 'sharp little 
thrusts' maybe either a desire to do that to oneself or others.  Hope food for thought
