I have a very complicated patient I have seen a few times and he has improved a lot and progress is being made. It is mainly on the M and E side. I am going to reproduce some of what he wrote in an email in the hopes that someone maybe able to lead me towards the next remedy!!! So far he has had Carc and Bar Carb.
I had asked him to describe the tension he feels:- (He has a first class degree in maths)

If there is something he wants to do e.g. sort something out on the car or design something practical for his house (he lives with his twin brother) he learns how to do it.
When he talks he often has to take deep breaths as he goes along and this is one of his problems. He is 30 and has never had a job. His social phobia has improved dramatically since the homeopathy. He has a problem with EFT.
Any ideas will be much appreciated

Hi, Rochelle.
There's some definite perfectionism here, it looks like. You might want to ask him to talk more about why he needs to understand everything, what he feels might happen to him if he didn't - it could give you more insight into the underlying "delusion" or fear.
The fact of being a twin can be very significant in a person's psychological patterns. Obviously, you could also use a rubric indicating competitiveness.
The feeling about "being about to burst a blood vessel" is an interesting expression. Roberts has the rubric:

Might be worth looking at a snake, or Nux-v?? Arg-met, Thuj, one of the Kalis, one of the Ferrums?? Depends on other symptoms and the physical generals that seem most dominant, active, and complete.

I remember you've mentioned this client before. It will be interesting to continue to learn about how he does.

Here follows my analysis of your case:

Breathing affected ! Got it. Everone knows IGNATIA sighs. Taking a deep breath is very close to a sigh. IGN is 4 in the Complete 2008 at RESPIRATION - DEEP - desires to take. Lets one look somewhere in a smaller repertory:
Boger-Boenninghausen, Respiration; DEEP; INCLINATION TO TAKE AND. BREATH; (43) : IGN the only remedy grade 4 !!

Materia medica:
As if about to burst
- Pain as if head would burst from talking or speaking loud, amel. sitting still, reading and writing.
- Pain as if intestines would burst, like cardialgia, extending into throat, in morning when lying on side, amel. lying on back. (T.F.Allen's Handbook)
General tightness
- Tightness of the chest. (Hahnemann)
- Tightness around shoulder and nape of neck. (F.J.Master)
- Tightness in chest, face and jaw. (R.Morrison).
The fact he mentions the blood vessels is not important because he gives on interpretation of his real feeling because he is intellectually-minded.

I should bet my shoes that the remedy is IGNATIA. I should myself give a minimum dose 30c (3 granules or even 3 poppy seed-sized globules as did the old masters...) dry once under the tongue during the crisis, better if the crisis is intense, on one hand for scientific purpose to see a spectacular vanishing of the symptoms, on the other hand to convince definitely the patient of the effeciency of pure hahnemannian homeopathy. If the patient is not hypersensitive to medicines in general there is no risk of aggravation.
Dear Rochelle, I think the dose will clear the crisis but other crisis will come. A deeper remedy should follow, perhaps CALC you already gave with betterment. After the crisis is over collect all the symptoms not belonging to it, especially the ones coming just after the crisis is over. You have to follow in the right succession of remedies.

enough to just know what you did, I felt I needed to know why you

Thank you Rosemary for your interesting pointers which I will research carefully. Yes I have mentioned him before and socially he has done remarkable well in the last 3 months. He is now driving himself everywhere, has gone clubbing, has been to a massage parlour (!!!!!), tried voluntary work and is now doing an accounting course. These are progressions that he never thought possible. If anything he has gone at too fast a pace trying to fit 10 years of wasting youth into a few months. I have to say I am surprised he is still coming to me. He never makes a further appointment when he is with me but a month later when it is due. The physical stuff really is just the breathing as his energy is much better and he hasn't really moaned much about aching feet like he did the first time. The breathing I see as a psychological think but I get shouted out if I dare mention any relaxation techniques!!
I never really thought of Nux Vom - but now you have put that idea into my head I will give it a look with him in mind. Thank you for the rubric which I would never have thought of!!!

Thank you so much for your analysis. Ignatia actually comes quite high in my rep (not as high as the Nux suggested) because of the rubric Monomania which I only hinted at but if a big part of the case. I shall certainly consider everything you have said.
I have not given him Calc but Carcinosin - was the first remedy that started him on the road to improvement. The breathing got a lot better but when repeated when it reared its head again did nothing. There is of cause a lot of grief in Carc. I gave the Bar Carb because I have had a case similar to this before and Bar Carb sorted it.
He is scared of taking the remedy and he takes it in water as I told him it was milder to do it that way. He also prefers to take it at home not infront of me in case he goes into a panic like he did the first time. (Well he called it a panic but it only lasted a few seconds!!)
The causation of a lot of this is mental abuse by the father (domination) in childhood and mollycoddling by the mother.

does he fear humiliation if he doesn't know or get something?
as a twin, what is his relationship with his brother?
does he feel invisible, or some such type of feeling in relationship to his brother?
what is his brother like? does he have easy success that may trigger this guy?
was he the proverbial 90# weakling as a child needing to find an arena in which
to excel?
did he have experiences of inadequacy relative to his brother when growing up?
he doesn't seem to have any love for learning from your notes, but a very ocd type
of behavior so it seems critical to understand what his obsession is about?

Dear Tanya,
Thanks for your thoughts. I have not met his twin but there is a competitiveness between them and I think if one feels something so does the other. He has spoken in the past about hating his brother although he lives wit him and feels he needs someone there. His brother also does not work but I get the impression that he is far more socially adept which as you correctly surmised does upset my patient.
Re obsession - he often has an obsession on the go hence my use of the rubric monomania. A recent obsession was emergency vehicles and he found YouTube footage of this to watch.

So also, "ailments from domination" and "fear sudden" are rubrics to consider. It occurred to me when you posted this originally that he was having panic attacks, but I didn't include that in my previous suggestions.

Thanks Rosemary. The ailments from domination was the main reason he got the Carc as his first remedy. I like the Fear Sudden. I hadn't thought of that one.
