Re; Confounding its confusion but specifically to discriminate the modalities [texture, color, odour, size shape etc] of one, two or more or between them e.g. state, objects, conditions etc.

Examples: confusion between night and day, between small and large, between sweet and sour, between black and red, lying or sitting, eating or fasting etc.

Rightly said it is found in schizophrenia, personality disorder, organic brain disease [SOL etc.], metabolic disorder [diabetic ketosis], meningitis, any thing causes irritation of Central Nervous System [CNS] etc.
