need distinction explained

when boeinghasen talks about throbbing, page 930..it is separated into
internal and external. What does that mean. internal would be throbbing felt
in the heart or stomach and external would be near the skin? Its actually in a
lot of rubrics...as in tickling in internal or external parts. Anyone know
what internal and external are referring to? I have a throbbing in my
finger...is that internal or external?
