Hi,
I have a case of a woman in her 6th month of pregnancy. She's had a dry cough for the last month or so, no clear causation except possibly returning home from hospital, where she said she felt better as she felt she was being taken care of. The pregnancy is after several tries, older single mother. In general the cough is dry, but in the morning it's accompanied by something she's not sure whether it's expectoration or actual vomiting, of copious amounts of a white coloured fluid. Sometimes this is accompanied by pain in the shoulders and upper back and a pain in the chest that she describes with her hands and with words "as if someone is grabbing at me". (the back and shoulder pain was there before the pregnancy). Perspiration at night, sometimes has to change her nightgown. Has not really had nausea during the pregnancy. She went into hospital because she had been bleeding.
In addition, since she left hospital she's had occasional pain in the lower right abdomen, she can't describe it, must bend forward. The doc says it's Braxton-Hicks (always sounds like that's rhyming cockney slang for something unmentionable.. for those who know cockney slang...)
I prescribed Silica but it hasn't done anything. Wondering in the direction of nat-m, or even puls or bamb-a (desire for support). Any ideas? The doctor wants to give her an antibiotic as it's been going on for four weeks, but she doesn't want to go that route. The repping I've done so far has not been helpful - so many remedies...so little time...

The pain in lower right abdomen is common. Usually it is round
ligament pain (brief, sharp stabbing pain or longer lasting, dull
aching pain). Ask if it is worse rolling over in bed (answer would
be yes). The round ligaments surround the uterus and stretch during
pregnancy. It can occur on both sides but more often presents on the
right.

It can also be indicative of a UTI but I'm sure they must have
tested for that.

However, did they do an ultrasound of her appendix or otherwise rule
it out? Appendicitis can happen in pregnancy and the appendix is
displaced.

I would consider Kali Carbonicum. Is she anemic?
Weak, weary, often anemic in pregnancy. Sweat easily. Desires
company - averse to being alone. Vaginal bleeding during pregnancy
with pain in back (although usually spreading down into buttocks and
thighs). Cough dry, hard, with vomiting, worse in morning. Chest
pain with a cough. Must sit up and lean forward to cough to get
relief.

thanks for responses. Could appendicitis be going on for a month? She says the pain is on and off, not every day. It actually seemed possible that during the silica there were less occasions of the pain.
Kali-carb did show up as a possibility for this patient. I'll look into it again.

Can you describe the expectoration in more detail - is it stringy like you'd get with kali bich or thin and watery like with nat mur

Could the coughing be causing the BH?

I was kind of thinking Coccus but would need more info as hundreds of rx have this type of cough and expectoration - maybe causticum

If the 'grabbing' sensation could be the same as 'grasping' and is in the heart region then I would use that rubric

She is being seen by an MD as well, so that is covered. The expectoration is white and copious, not described as stringy, more vomit-like. Not thin and watery, but possibly slightly viscous, like egg white. I considered Nat-Mur, it was the first remedy I gave her before the pregnancy. Didn't seem to touch that much at that time, but possibly is worth another look.
She demonstrates the grabbing sensation by raising both hands to her chest, a little below the collarbone on either side of the sternum.

Do you mean she is grabbing her chest when coughing like Bryonia does (acute of nat mur but I don't think it has white expectoration, usually red). I still think you need more sx or modalities with this as there are too many rx to consider and they need to be cut down.

If it is actually coming from the stomach and not the throat or lungs then you could use the rubrics that pertain to coughing finishes with vomiting or vomiting with cough etc. Joy
