a new development in the Malaria case (a beautiful cure)

Well,

It looks like the "beautiful case" ain't so beautiful (yet) and has developed a glitch. Any guidance is welcome.

Firstly: the patient is far away and I can only get info over short telephone calls or text messages. I send the rx via courrier and wait for results.

Background:
This is the "beautiful cure" case I posted recently.

To remind you: Male, 16 yrs, bi-monthly periodic cateleptic fits diagnosed as "epilepsy" after malaria suppression-cum-quinine poisoning 1 year before. No previous history of any epilepsy. I'm supposing this was a cerebral (malignant) malaria that the boy suffered.

Treated with 3 doses of China S. 30

Update:
From July when treated until 4 days ago (3 months in all) there have been absolutely no symptoms. The patient appeared cured by all observations- mental faculties returned, the boy returned to school, no more fits reported.

Four days ago I got a call from the father that at around 7:30 pm the boy fell down in a cateleptic fit (limbs stiff and still, eyes closed) and didn't get up for 1 hour.

The case repeated itself again the next night - fell down at about 7:15pm and again last night.

During the second fit is when I spoke the most to the father. The boy, by that time, had been unconscious for more than an hour. During that period, he awoke for a few seconds and went unconcious again. The father didn't consider it as sleep, but still under the effects of the catalepsy.

("Catalepsy" is _my_ description of the stiff limbs, non-movement of the fit. There is no appearance to a typical epileptic fit with convulsions, etc.)

During the boy's unconsciousness I asked the father to check the eyes which were closed. He only said that they eyes "wanted to stay shut" and turned upward when he flashed a light in them (on my instruction). Its not clear if the pupils reacted, but i think so.

I didn't know what else I could ask to dig out any sx. Apparently there are no mentals/ emotionals to have triggered this. No exceptional physical strain either.

The father didn't have any china sulph with him, but had some of the other "backups" (not combos!) that I sent along as possible alternatives. So about 5 minutes after he gave ARS A 30. the boy awakened. It took him another long period of time to get out of confusion and normalize.

The next day he took the boy to the hospital to check for malaria & meningitis, both are negative. He still had a (3rd) fit that night despite apparently getting ARS A throughout the day.

My differentials are coming up like this:

Ars A = periodicity, coma, malarial (origins)
Cedron = <7 pm, periodicity, coma, malarial
Opium = catalepsy, coma after epilepsy

Other "top runners" are in the below repertoization: gels, nat m, chin s
And could Crot-h be _The One_??? (see last rubric below)
I've sent along Ars 200 and am waiting for results. But in the meantime any homeo or medical-clinical guidance to further this case would be appreciated. Could there be any medical causes that I haven't considered?

For easier reference I'm copying below your original description.
So--he got malaria, got medicine, and now has cataleptic attacks.

Maybe it's important whether this was "malaria suppressed" or whether
it was a toxic effect of the drug? Can you get the name of the
medicine, and we can see whether it has any known side-effects that
might be relevant?

OTOH--You said he has not yet had any further dose of chin-s? Wouldn't
that be the thing to try first, and if 30 no longer acts, try moving on
to 200?

Margaret Tyler has Nat.m as a remedy for convulsions after quinine - it is in Synthesis and the
reference says it is from her 'Drug Pictures' book

Actually Nat.m comes up second in a general rep as well

The type of convulsion the boy is having sounds like it is a 'tonic' one, without the 'clonic'
aspect.

Please note though, that there is also a type of condition in malaria called 'Cerebral Malaria'
where there can be motor abnormalities such as decerebrate rigidity, as well as decorticate rigidity
(arms flexed and legs stretched). You may want to look that one up and see if their is some
association with your case.
But I get the impression that this occurs whilst the malaria is in full swing.

So, in this sense, due to either cause, there may be a brain lesion that is causing this problem.

also, here are some snippets from Banerji's clinical work that may be of interest -- it is more the
comments in the notes after his totalities that one can learn some important info.

The following is in the Mat med of Cannabis Sat. 200th potency. I am aware that the boy does not
have ascites, however, it is the comment on Puls that caught my eye.

GROUP B - 1
Ascites after abuse of quinine or quinine products.

Dose Once at interval of two days.

Note As antidote to the effects of quinine, Pulsatilla 30 is no doubt very good, but ascites does
not respond to it. Moreover, ascites as a side-effect of quinine. is the characteristic symptom of
Cannabis sativa 200. The detailed symptoms of ascites may consist of many other symptoms, but when
there is a history of using quinine before the onset of ascites, we should use a few doses of
Cannabis sat.200, which may cure the disease.

GROUP D - 1
Intermittent fever.
Morning temperature between 96'F and 99'F and evening is 102'F to 106'F as commonly found in cases
of malaria.
Fever comes with chill and shivering.
Sweating while the fever remissioning or decreasing.
Malaria or influenza.
Spleen often found enlarged in cases of longer duration.
Other symptoms like : frequent thirst for small quantity of water,
restlessness (not that typical as in Group C - 1), fear of death, anxiety may
be present.
Dose Once daily in the morning or a single dose also may cure.

Note Best results were obtained when Arsenic 6 is given early in the morning and Eupatorium perf.
30 is given at 9 p. m. In cases of malaria, where malarial parasites are found in the blood, if such
alternation of Arsenic 6 and Eupatorium perf. 30 has failed, Chininum sulf. 3x or Viton (a
preparation of Vitex n. 1x) has succeeded. The so far known Allopathic Quinine sulphate or chloride
or phosphate is purely a Homoeopathic drug. This is evident from the first experiment of Hahnemann
with Cinchona, the Allopathic drug, which he used as Homoeopathic, for ague or malaria then, when
for nine years he had used it as it was same in Allopathic and the potencies or present form of
mother tinctures were not developed by him till then. Quinine salts as mentioned are also successful
Homoeopathic drugs, but unfortunately its use has been monopolised by the Allopaths, and the
Homoeopaths are prejudicially not touching it in fear of losing their orthodox religion dogmatically
and due to sheer ignorance. In the advanced cases of malaria it is quite Homoeopathic to use the
Quinine salts like the sulphate or chloride or phosphate.

GROUP P - 1
Intermittent fever of malarial type.
Patients feels heat once to remove covers.
There is hot sweat.
There is vertigo and headache when the chill begins, shivering of the entire body may be noticed
with sudden pain in the parietal bone as if hurt and broken.
The time for beginning of the chill is mostly at 11.00 a.m. and in the following day at 4.00 p.m.
There may be shivering with the chill which may be also in the evening or sometimes in forenoon.
Drowsiness and fatigue in all limbs.
Thirst is noticeable during chill.
In some cases, there is drawing in all joints and great heaviness felt in head and whole body
before the chill begins.
Peculiarly when there is external heat of the body, the patients feel chilly internally, especially
it occurs in the evening.
Such intermittent fevers respond to this drug even when there is misuse of quinine.

Dose Twice a day.

Note In such cases if it is in the early stage of malaria, Eupatorium
perfoliatum 30 given every three hours cures the symptoms
and the diseases. There are many cases of intermittent fever
with the above mentioned symptoms where malarial
parasites are not found, or no correct diagnosis could be
arrived at, they are cured with Calcarea carb.30.

Dear all,

Thanks Robyn & Shannon for your imput. Both Gels and Nat M had crossed my mind but for my quirky reasons I felt these might be too "mild" for the job at hand....
You have brought up good references for them, they are back on my "A" list....

In the meantime further developments.
As it happened, the boy took poison (pesticide) to kill himself last week. He survived the attempt and is in the hospital now. He will get out soon.
It happened the morning after the 2nd fit (which the father represented to me as the 2 & 3rd fit) When the father went out to pick up the medicine (ARS 200) from the delivery point, the boy took the poison and they rushed him to the hospital. He never got the Ars 200.
All he had was a few water doses of the Ars 30 that was with the father at the time.
He has not had, until now (about a week later), another fit episode since the Ars 30. (which, in any case is not definitive, since we don't know why he had a fit nor why he hasn't since)
On the other hand, I finally got some "mentals" out of it which have enabled me to think more about a remedy!
Well in the meantime... the boy is in the hospital and recovering from the poisoning. He's taking drugs for the poisoning, and I told the father to continue Ars 200 for poisoning as well. The boy regrets having made the attempt. He has not had a fit since.
"Desire to die" would seem to characterize the boys feeling's according to the father. He felt so despondent after the 2nd fit, he felt there is no hope of getting better, it would be better to die.

More, I was surprised when the next day I continued of my study on Aranea Diadema I found this in Murphy's:
HOMEOPATHIC -- All spider poisons powerfully affect the nervous system. (Tarent., Mygale). Aranea diadema is the remedy for the constitution favorable to malarial poisoning, where every damp day or place favors chilliness. ...
All symptoms of Aranea are characterized by _periodicity and coldness_ and great susceptibility to dampness. ....
Swelling of the spleen after malaria suppressed by quinine, malarial attacks worse in wet rainy weather, severe attacks of neuralgia after malaria suppressed by quinine....

CLINICAL - ... Catalepsy. .... Intermittent fever.... Malarial cachexia.

Modalities - ..._Exact periodicity._ Periodic recurrence at the same time of neuralgic pains and cold shivers. Worse from mental strain, especially of head pains.

MIND - Nervous. Despondent, longs for death. Alternately, happy and depressed. Great inner unrest, irritability, fear of death. ...Confusion of mind, better by smoking in open air. ....
So I've prescribed Aran D. 30c for 3 days and to see if there is a change in his mind and maybe sx.
(30 is what I have in the dispensary)

and ... yes, he is now living in a wet climate...
