milk teeth won't fall out

I'm trying to find a remedy for a child whose milk teeth are stubbornly staying put. She's almost 10, small for her age. Some of the adult teeth have come through, but the milk teeth seem to wobble, almost fall out, and then firm up again.
She's received Lachesis as a constitutional, and it has helped her a lot, seems to be the main remedy that works whenever she comes down with anything. She's extremely talkative, sometimes very outgoing, sometimes not, doesn't have much of an appetite for anything except sweets. She's very warm, affectionate.
It seems that Silica would have connection to the teeth - but it seems to have absolutely nothing to do with the constitution so I hesitate to prescribe it. She's received Calc-Sil in the past (used to have a lot of dreams about dead people, talk of dead people), but it didn't seem to touch the teeth. Her toes are somewhat crooked, and some of the nails are slightly concave. The teeth that are growing in also seem to have independent directions of their own!
Any ideas as to rubrics/remedies?
