When we prescribe on the mind symptoms we cannot always try to encompass the
ultimated pathology. The "Kentian" goal is a remedy for the "patient" and
the "disease" as a vector sum. From the horse's mouth:

"...The difficulty in prescribing for patients with such altered tissue -
cataract, hepatization (in pneumonia), induration of glands,
arterio-sclerosis, fibroids, cancer, etc. - rests in the fact that when
these tissue-changes occur, the symptoms on which a prescription should be
based - the symptoms of the patient - have disappeared. The symptoms
present at the time are symptoms of the pathology. If the symptoms
that preceded this condition can be learned, and considered together with
the later results of disorder - the pathological tissue - it may be possible
to select a remedy that is sufficiently related to both the patient and his
pathology, to effect a cure of both, provided always that the reaction
and vitality of the patient are sufficient to permit the resolution. "

This article is four years from his death -- and in it Kent does not discuss
finding a separate remedy for the totality of the wholism of the pathology.
This would indicate he is an adherent of only the overarching approach.
Many would argue (in the ongoing discussion that frequently occupies this
discussion board) that Kent's tendency to put all his eggs in one basket is
a departure from Hering, Boenninghausen, and Hahnemann himself. It
certainly is only ONE method of choosing a totality on which to prescribe.
Count Boenninghausen's hexameter, and Paragraph 153 attest to that. David
Little expounds eloquently about this in literature on his website.
Sometimes we must approach the pathology first, then the remedy of Kent's
"patient" "returns" or becomes known. This is a second approach which
breaks the case down into components: as yet dissimilar disease for which an
overarching remedy which includes the "patient" cannot yet be found; and
the "patient" (mind symptoms, thread of delusion, sensation etc).
An overarching rx is for the sum of miasmatic distortions, not aimed
directly at the miasms of the psoriasis wholism in particular. Both
overarching -- (and direct methods of which we weaned by Kent, Vithoulkas,
Sankaran, etc are much less accustomed) are potentially contributory to
eventual cure of the psorasis.
Mixing the ongoing states of the patient as a whole with addressing the
organic pathology is our usual (and Kentian) method. But in some cases
where the dominant miasm underlying the organic pathology is stubborn
regardless of the rx that is findable for the "patient" we may need to treat
the miasmatic basis for the psoriasis as a wholism on its own to decrease
its virulence. This may bring out the picture for the eventual lasting cure
-- by one remedy of the sum of the distortions of the whole pt -- made
known by diminishing the dominant miasmatic component relating to the
psoriasis itself.
What we can conclude in some cases is that a successful "lesional" remedy
for the psoriasis IS/WAS the overarching rx on the surface of the case,
whether we regarded it as such or not when we prescribed it. A remedy we
know less about in data can be overarching even if it seems to be focussed
and small totality -- only because we have little of the "rest of the
picture" of the remedy in spheres beside the pathology on which to base the
prescrip.
Other times, it is simply the rx for the pathology, without inclusion of rx
for "the patient". This is the case with Dr. Ramakrishnan's approach to
pathological prescribing, Eizayaga's approach, Somenath's research,
Chappell's remedies -- prescribing for advanced pathologies in general.
These are approaches aimed at the wholism of the totality of the pathology
within the pt which is what needs urgent attention -- when no overarching
remedy is expedient or certain.
Naturally being aware of approaching the case like this does not mean to
overlook seeking the overarching remedy first and ongoing. Eventually it
can be found, but it may take longer than the patient hangs around unless
the urgent complaints are dealt with. As J.T. said, if that overarching
remedy can be found, it is a boon. If it cannot be, however, we need to know
how to cure the case another way.
