INFO on this condition please

Can anyone give me some info about this condition please, I have not
come across it before?

The patient has two this size and other smaller ones coming up.

I have permission to post

The site of the other largest one, about the same size as this one is
on the right hand ankle.

If anything the one on the ankle is slightly larger. There are others
coming up on the right lower leg but as yet they only appear as skin
discolouration about the size of a small finger's nail

Neither at the site of an obvious injury, other than the possibility
of vaccination but that would have been over 40 yrs ago

Then I would suggest that it is erythema nodosum, this can be a sx of
some other underlying condition as well. Joy

Thanks Joy,

yes, EN is a possibility but from what Iv'e seen this would not be
typical.

It obviously betrays an underlying condition, but I hoped someone
would have actually seen this before and could give me a bit more.

No one come across this before on their travels?

EN tends to be less circumscript, indeed, and has a preference for the lower limbs.
However, I (personally!) do have have two such similar spots, one on the backside of my upper right arm, near the shoulder, and one on my lower right tibia, both smaller than the ones you showed, and both have the habit of getting redder, and desquamating, and a bit itchy, a few times each year. As a rule, however, the spots are rather more brownish and I live with them unnoticed.
In general, I would judge my miasms as 2/3 sycosis and 1/3 tub.
Are these spots troubling your patient in any way?

Hi Hennie,

here are some comments from the Px about these:

"Thanks Simon. I looked but the nodules have never been painful in
themselves. Also more recent ones are simply suncutaneous swellings,
not discoloured at all. One of them came up on my hand within a few
hours, ie wasn't noticeable at one hand-washing , was at the next (!)

I think we discussed Granuloma annulare before, but if not, might
that be a possible?

None of which is much help since there doesn't seem any clear cause
for any of these things."

There is clearly a bigger picture going on but I'm a bit in the dark
here until I can get these pinned down
