Azoospermia!

Hi Vivek,
Only once I had an azoospermia ("zero" sperms on repeat analysis) responding
to homeopathy
and the count increased up to 8 million/ml. Besides that I had no luck with
any azoospermia
but had successfully treated many severe to mild oligospermia cases.
Now to add to what Soroush had said..
You may proceed in the following manner (systematically) in all such cases.
1. Ascertain his sexual.. knowledge, drive, motivation, potency,
performance, satisfaction.
2. Physical examination of the person as a whole (to rule out genetic
disorders).
3. Physical examination of genitals - penis, scrotum, testes, epidydimis,
seminal vesicles, prostate (needs some expertise as it is a per-rectal
examination).
4. Semen analysis - sperms count, motility, morphology (what are the
abnormalities),
viability, pH, viscosity, fructose content, pus cells (all are important in
the order).
5. Hormonal assay (Testosterone, FSH, LH, Prolactin, SHBG).
6. Testicular scanning.
7. Doppler study of scrotal vessels.
8. Testicular biopsy.
9. Karyotyping (chromosomal study).
10. Psychological analysis of patient and partner.
In azoospermia also all the above steps are to be taken (except semen
analysis if
it is already repeated at least twice and the last as it is very unlikely to
have
psychological cause to total absence of spermatozoa).
The probable causes of azoospermia may be (in order of their prevalence and
gravity
of a good prognosis) any of the following...
(i) Post Traumatic/inflammatory lesions to testes
(ii) Varicoceole
(iii) Hormonal problems
(iv) Chromosomal abnormalities
(v) Degenerative lesion of testes.
From what little symptoms you have given it seems to me like a chromosomal
problem.
You can easily suspect it by a physical examination. For confirmation seek
the help
of a 'geneticist' who will do it by blood tests (karyotyping) even though it
is a little costly.
