pigmentation

Can any body tell any cause for the pigmentation of the feet and ankles.
The pt is getting macular patches/ mostly spots from 1mm to 3mm diameter
macular brown patches in feet and the ankles.

The pt is over wt and having slight hypertension. other wise ok pt is
just 33 yrs old male i suspect some liver abnormality but not able to
diagnose.

Can anybody suggest any probable cause or condition which can produce
this type of pigmentation only in feet and ankles? Will over wt produce
this?

fungal infection / treatement a fungal cream for 2 weeks and then sun
exposure to re form pigment.

Here is another option

Venous Stasis

Generalized increased pigmentation occurs for a variety of other reasons. Dark patches of skin occur
about the ankles and lower legs in persons who suffer from Venous Stasis. Venous stasis is caused by
an accumulation of fluid in the lower extremities. This is due to poor venous return of blood to the
heart. Venous blood flow back to the heart occurs by way of the veins in the feet and legs. Venous
stasis is associated with varicose veins that do a poor job of returning blood to the heart. As a
result the blood flow is slowed, becomes stagnant, and fluid accumulates in the ankles and lower
legs. As the fluid accumulates in the lower legs, the small and medium-sized veins break or leak
fluid into the tissues. As blood cells break up in the tissue, they deposit the iron that is part of
hemoglobin in the blood cell. The iron stains the skin causing a light to dark brownish appearance.
With time, the skin and subcutaneous fat becomes thinned and will break down creating weeping venous
stasis ulcerations. At times, blistering will form with a clear, watery fluid weeping from the skin.
This condition requires professional attention by a physician.

Treating a fungal infection by external means is not such a good
thing...
Aside from the question of "healing versus suppression", at the very
least, that approach allows the *internal* yeast imbalance to continue
unchecked, which will have further and worser consequences...

Thank you for the response.

I had send the pt for a abdomen scan and report says the pt has fatty
liver and (gall bladder distended) but there is no any calculus and all.
Pt is obese. 93 kg and height is 5.7" and is of a big frame. He
developed this abdomen recently may be due to not having any exercise,
sedentary work and lot of travelling. Hypertension is 150/90mm of hg.
This bp is seen only after he put on wt.

No any cardiac problem or symptoms are indicated till now. Pt is a non
smoker and do not touch alcohol.

The resent obesity and hypertension is not well explainable. As he is
not overeating or having any oily fried food.

The pigmentation is not generalised even on the ankle it is paplar and
like small dot's i don't have much clinical exposure to tell with
certainty that it isnot of venous origin ie statas and all. One thing
is sure it isnot and skin condition or fungal infection. The colour is
of that of lichen planus but it is not lichen. The pigmentation does
respond to liver remedies.

Hi Dr. Nandana,
Are you aiming to treat this with single-remedy homeopathy? If so, are
you using a "totality" approach, or looking at organ remedies? From
what you've posted so far about it, I'm at a loss where to even
begin--maybe others are too, and that's why you're getting scanty
response.

If you are looking for a "constitutional" / totality based single
remedy, I have some questions to ask, but if that's not what you're
trying to do, they probably wouldn't be useful... But in any case it
might help to know what else was going on at and before the time when
the big(?) abdomen suddenly began? Any changes to his routine or
circumstances, and did any other symptoms end or begin at around that
time?

Are the colored dots on his ankles raised, or flat? (Does "papular"
mean that they're raised?)

Sounds like chloasma to me as a starting point and in a male could be
caused by thyroid dysfunction which could explain the overweight. Best,
