Sleep Apnoea

Actually this cannot be told as a typical sleep apnoea as it is very
very much related to the job.

The first attack was on the day he lost the actual promotion to a
managerial post. All his relatives told every body they know that he got
a promotion - when he had just told that he may get as he is one of the
person who are short listed and as he was short listed other ppl also
stated to fear him and all... But he know he has only a very small
change of promotion. In the tough competation the most ineligible or who
had the least change got. And the pt is not against the decision.

All his relatives stated to call him and congratulate and ask him treat
the same minute when he knew the fact he is not selected and he lost and
the person who called had know the "happy news after so much of travel
from mouth to mouth. He was to face 1. new problem of explaining to all
that he did not get promotion
2. Has to do the same old job which has a very high chance of insecurity
for amount of income he may get each month 3. Got a feeling that he is
not good - not up to the expectation of his relatives or they are over
estimating his position and putting him in a situation where he feels
realy sorry or more sorry than he would have been even if he did not get
there. And he is not in a position to tell them not to tell all his to
other ppl before he gets the post.

Pt is not afraid of dark only since this loss of promotion change he
gets his acute attack of suffocation with in one hr of going to sleep
( it is more or less in in the mid of the first sleep cycle ) He gets up
in deep sleep sits up in bed of ( wife who is a disturbed from every
single small sound from leaking of the pipe to the running vehicles on
the street) fully sweat and flushed face pulling her alone with him as
he has hold her hand tightly and tell to on the light if he had to go to
sleep again he needs light due to fear of second attack - ( he has never
got an attack on holidays eves) Last 5 mnt almost ever week he has an
attack the must severe was the attack fist time on that day he was very
very afraid went to toilet with and uneasiness he was not able to
describe Even after so many attack he is not sure why he wakes and why
he is afraid to go to sleep what is seeing exactly - is he waking from
stooped breathing which gives a dream or a dream which cause him a
attack due to the severe fear In the morning he know he did some thing
but does not know what he did or what he talked.
He is balding now a days has reduced his wt that he used tobe some 2 yrs
back. but job tension has increased to 200% in this period.

Desire non veg but produce constipation has burning in anus after hot ie
spicy food he likes the most. Has some particular desire in his food
want fresh food but still crave hotel food. Wont eat food made in the
morning in the afternoon. does not alike new food ideas or dishes he
wants it in his style. Physically very lazy does not do any physical
work getting even one floor through stairs will make him pant due to
such lack of movement. Head oil bath produce slight body ache on shoulders.
I feel Silecea, Phos, Nux vomica.and even lyco but not in any way able
to make a last opinion.

Sorry i dont know sambucus and all as a deep acting medicine.

From this information (below) it seems like the triggering etiology could be
a form of public humiliation. Premature accolades that were unwarranted.
(Or fright from lack of money because the promotion was lost, complicated by
humiliation). These only the pt himself (not wife) can say.
Is the hair falling only since the promotion incident w/possible
humiliation?

Was the weight of this person stated as 112k = approx 246 pounds -
that's obesity and could be thyroid (spongia) and maybe goiter causing
the apnoea. Spongia also tubercular rx and I tend to think of 'most' of
them being really thin but if there is also a thyroid problem it can go
either way - spongia has a heavy body and easily exhausted and thus can
be lazy, this hints at an under active thyroid.

When you have a physical sx such as suffocation that could be part of a
theme throughout the whole case it is up to the practitioner to seek
that out and whether to use 'fear of suffocation' as a leading rubric
and then as to whether the fear of suffocation is from a physical cause
or/and an emotional one. All rx mentioned so far are in this rubric.
