I know that I am a little late in posting on this thread, but I fell
behind with my emails from last week. I wonder if you are still
interested in rubrics for your case. I think the perfect approach for
analyzing this case is through Boenninghausen method since you
covered all the aspects of the chief sx related to location,
condition, sensation modalities, as well as some concomitants. I used
the Repertorium Universale, which allows this kind of analysis and I
think the results are quite interesting. Rubrics chosen:

LOCATION, CONDITION:
Ear, sides, right
Ear, phenomena, inflammation
Ear, phenomena, discharge purulent

SENSATION
Ear, phenomena, stopped sensation
Ear, phenomena, itching burning
Ear, phenomena, pain, aching

MODALITIES
Ear, phenomena, pain, air open aggravates
Ear, phenomena, pain, air , cold, aggravates

Based on these more general rubrics I selected the remedies that came
up in 3rd or 4th degree (the grading system in Repertorium Universale
has nothing to do with the intensity of the symptoms but they are a
characteristic of reliability). The rubrics contain quite a large
number of remedies and I made a selection of those remedies that
appeared more frequently. They are: Acon, Bell, Calc, Graph, Hep,
Kali-c, Lyc, Merc, Puls, Sep, Sil, Sulph, Thuja.

Some other remedies were not consistently present in most of the
rubrics, so they were excluded: Aur, Bar c, Bry, Caust, Fl-Ac, Lach,
Mang, Nat-m, Psor, Tub, Tell.

The relatively large group of remedies was carried through the next
set of rubrics, which cover some particular details of the symptom
(condition), for differentiation:
Ear, phenomena, discharges, general, offensive
Ear, phenomena, discharges, excoriating
Ear, phenomena, discharges, thick
Ear, phenomena, discharges, yellow green
Ear, phenomena, eruption, moist

As a result, the following remedies were excluded based on the poor
score: Acon, Bell, Calc, Graph, Kali-c, Sep, Sil , Thuja. The
remedies that came through in a sufficient rank for consideration
were: Hep, Lyc, Merc, Puls, Sulph.

At this stage I took into consideration the CONCOMITANTS as mean of
differentiation and I used the following rubrics:

Sleep, position, side on right
Gene, weakness, pain from
Mouth, dry, thirst with
Mildness
Hopefulness

This way I could rule out Hep, Merc, and Puls.
All in all, the case was repertorized on the following rubrics:

Ear, sides, right
Ear, phenomena, inflammation
Ear, phenomena, discharge purulent
Ear, phenomena, stopped sensation
Ear, phenomena, itching burning
Ear, phenomena, pain, aching
Ear, phenomena, pain, air open aggravates
Ear, phenomena, pain, air , cold, aggravates
Ear, phenomena, discharges, general, offensive
Ear, phenomena, discharges, excoriating
Ear, phenomena, discharges, thick
Ear, phenomena, discharges, yellow green
Ear, phenomena, eruption, moist

The following remedies resulted in the end:

Lyc: 17/ 38 – lacks discharges yellow green (but is present in the
rubrics Ear, phenomena, discharges, yellow and Ear, phenomena,
discharges, green) and weakness, pain from.

Sulph: 14/39 – lacks air cold aggravates, discharges yellow green,
eruption moist, weakness pain from

Puls: 11/31 – lacks pain aching, air cold aggravates, air open
aggravates, discharges offensive, eruption moist, sleep position side
on right, mouth dry thirst with

Hepar: 10/19 – lacks itching burning, pain aching, pain, air cold
aggravates, discharges yellow green, sleep position side on right,
mouth dry thirst with, mildness and hopefulness

So, you may want to check if there are any additional generals or
mental and emotional aspects of the patient that can help in
differentiating between Lyc and Sulph. Both remedies are present in
the rubric: Ear, discharges, chronic (Synthesis). The references are
from Farokh Master for Lyc and from Master and Boger for Sulph. The
fact that Sulph misses the aggravation from cold air gives priority
to Lyc in my opinion. I also checked the rubric for general
aggravation of ear sx from cold air in RU and Sulphur is not listed.

The final recourse to Materia Medica should help you clarify if and
which one of these remedies is the simillimum.

I started using Repertorium Universale only recently. Is there
anybody on the list who has more experience with this repertory? Your
insight would be really helpful. Thanks.
