Please consider that you may need to make some changes:
X = Decimal potency (dilutions of 1 to 10) (Can be represented as D potency in Europe.) CORRECT
C-= Centesimal potency (dilutions of 1 to 100) (Can be represented as DK and CH potencies in Europe.) CORRECT
M = Millesimal potency (dilutions of 1 to 1,000)
INCORRECT. M potency means that the C dilution (ie 1/100) has been carried out 1000 times)
CM = Millesimal potency (dilution of 50 to 1,000)
INCORRECT. CM means that C potency has been carried out 100,000 times
Also there is MM which is C carried out 1000,000 times
The normal series is Tincture, 3C, 6C, 12C, 18C, 30C, 200C, 1M, 10M, 50M, CM, MM
LM = 50 Millesimal potency (dilution 1 to 50,000 w/ 100 successions) (Can be represented as Q potency in Europe.) CORRECT

I do not want to deal w/this by sending my pts to a diff pharmacy. Everyone should be able to go to their neighborhood pharmacy and buy a rem with as much ease as buying any product.
That my pharmacy, which is a very good pharmacy, can dispense hom & salts incorrectly – repeatedly - means that this is going on in many pharmacies.
The last pt that was given the wrong potency, took the tissue salt before realizing that it was in the wrong potency. He called me very frightened. "Am I going to die!?"
This man, who wasn't feeling well, had to come back into Manhattan to get the correct potency. (The pharmacy exchanged the already opened bottle for the new one w/o charge and is giving him %20 off of his next order.)
It is the oldest pharmacy in the US and has had a respected reputation for a very long time.
I think it will be helpful to have an information sheet to give to the pharmacy and to be able to give it to my pts to give to the pharmacy that they use if different- so I started working on one.
Below is my beginning draft.
If anyone has the time and inclination please check for accuracy and clarity and if anything should be included that is not or something should be left out.
I don't think it should be more than one sheet of paper, front & back.
Also does anyone know of any great current links to add at the bottom for further info?

Homeopathy and Tissue Salts for the Dispensing Pharmacist
Remedies act differently in different potencies. Being exact with potency for a homeopathic remedies is as important as anything else dispensed at a pharmacy.
The potencies
The remedies are diluted (and dynamisized) with either lactose (by grinding - trituration) or water and alcohol (by shaking - succession).
X = Decimal potency (dilutions of 1 to 10) (Can be represented as D potency in Europe.)

Tissue Salts
Not actual "salt", the twelve mineral electrolytes are found in the body where they act to help normalize tissue processes and restore metabolic function on the cellular level.
Although the salts are not considered homeopathic, they are lightly "dynamized" and diluted like homeopathic remedies.
Also called "cell" salts, "biochemic" (chemistry of life) salts and "Schussler" salts, after chemist Dr Schussler who developed them.
Unless specified differently tissue salts are commonly dispensed in a 6X potency (see above under potency) The salts only come in a "X" potency, otherwise at the higher potencies they become, through dynamic dilution, a homeopathic remedy.
Tissue salts do not come in a blue tube.
Homeopathic remedies
Homeopathy is a scientific methodology for healing the "whole person (mind-body) that has its hypothesis in the works of Hippocrates, Parcelsus and other healers who recognized that a substance that produces symptoms in a healthy person, if diluted through a specific process, can cure those same symptoms or that "like cures like."
Homeopathic remedies are primarily made from natural substances, such as salt and flowers that have been so diluted that there is no longer any gross material left in the remedy. What is left is the energetic blueprint of the substance. Because of this dilution, homeopathy is especially well suited to highly sensitive people.
Through understanding an individual's clinical picture or "totality" the homeopath can match this individual picture with the therapeutic remedy picture.
* Each homeopathic remedy pill is 1 dry dose (or for making medicinal water solution.)
* Avoid touching the remedy. (The remedy can be transmitted by dermal contact.) A pill can be shaken into a cap then dropped under the tongue or into a bottle/glass. If any pills spill out dispose of them.
* Store remedies out of direct light and away from strong odors and heat.
* Possible antidotes: coffee, electric blankets, meds, dentistry, recreational drugs, strong odors.
Homeopathic remedies and tissue salts have no drug interactions with prescription drugs and do not interfere with the action of prescription drugs.
Remedies are FDA approved, prepared in FDA inspected sterile facilities, and listed in the HPUS, (the homeopathic equivalent of the USP.)
