Start with Chloasma rubric as this is what needs to be cured - her embarrassment came afterwards and is not a causative factor.

But isn't the embarassment a potentially individulising part of the
whole picture of what needs to be cured.

Cure the melasma and you cure the embarrassment. Yes it is part of the case, such as her reaction to the situation which could help define the simillimum but as all sx have to be ranked or graded this is secondary to the cause.
