I think Michel’s idea of Ignatia is a worthy one – and also have another thought.
The idea came from Sankaran’s plant sensations – of which the Anardiaceae family came to mind when your client mentioned ‘tightness’. The remedy that comes to mind from that group from your notes, was Anacardium.
Because my main work is as a psychologist, I see a lot of people with similar issues as your client has described – and I don’t fully rely on a homoeopathic remedy for the healing – but have found that in the cases where there is a high intellect and then disorders such as monomania etc.... a remedy often helps the rest of the therapy.
The tightness could also be ‘tension’, and it can lead to a feeling of being paralysed, immobile or stuck – and some will try to change this by becoming restless – but not always. They get frustrated because they feel stuck.
Some rubrics that could support Anacardium and a few other rems that could be used are:
Monomania
Thoughts – persistent
Fear of failure
Anger
Mental tension
Want of self confidence
Anxiety – respiration impeded (which leads one to take a deep breath to try and break the tension)
Ailments from being abused
General tension of muscles
When looking at some of the keynotes of Anacardium – and keeping in mind that you don’t always have to have the dual personality aspect with the cruelty part showing through for this remedy to be beneficial:
There are aspects in Anacardium as in Baryta Carb – eg.,the lack of self confidence, which is big in Anac.
Internal conflict from the need to prove himself
Stays in bad situation ; needs to prove themselves over and over
Students before examination: fear of failure; students who pressure themselves intensively
Anxiety about future
Irritability, fly into temper
Feels as if living in dream
Complaints from humiliation – being put down. Etc.........
It is also a good remedy for states of depression that are not suicidal as in the Aurum depressions. It also has the inability to think at times, when under pressure. The 200th potency is particularly noted.
Anyway, it was just a thought – as I don’t contribute alot to the list these days, and have been recouperating this week, and had more time to think and explore.

Thank you so much for your analysis. Ignatia actually comes quite high in my rep (not as high as the Nux suggested) because of the rubric Monomania which I only hinted at but if a big part of the case. I shall certainly consider everything you have said.
I have not given him Calc but Carcinosin - was the first remedy that started him on the road to improvement. The breathing got a lot better but when repeated when it reared its head again did nothing. There is of cause a lot of grief in Carc. I gave the Bar Carb because I have had a case similar to this before and Bar Carb sorted it.
He is scared of taking the remedy and he takes it in water as I told him it was milder to do it that way. He also prefers to take it at home not infront of me in case he goes into a panic like he did the first time. (Well he called it a panic but it only lasted a few seconds!!)
The causation of a lot of this is mental abuse by the father (domination) in childhood and mollycoddling by the mother.
