Verat.a is good as Dale pointed out.
I think Calc has many of the symptoms. In fact most of the carbs
have the symptoms. Coldness, Excess bleeding, tumors etc. Hot blood
is Bell..
Kali.carb has the diarrhea, coldness, starting from the first
pregnancy symptoms.
Amm. carb, Kreos also look good.
I would probably start with Kali.carb. Looking just at the bleeding -
Trillium looks good.
This case has so many modalities and concomitants that with some
more study to differentiate between remedies prescribing should not
be a problem

Hi Hahnemannian and all,
I'm curious why the mentals aren't being mentioned at all? I know Vera
said she's had some success with the mentals but without touching this
set of symptoms, but wouldn't the mentals still be relevant in
differentiating among possible rxs?

I did not see about mentals in her post. She mentions Constitutional
and for me that means generals.
You probably know by now, from many of previous posts, that I am not
a "mentals mainly" or "mentals only" kind of prescriber.
For me mentals are but one of the generals - one of the "I"
symptoms -

I couldn't find a rubric connected to menses with this symptom.
She's sensitive to wheat and dairy - but from my reading I find that this is common amongst women who have fibroid conditions.
Right now I think I'm down to Bell and Sabin - although I'm checking into Trillium as well. Also following up with dietary and supplement recommendations, including Vitamin C, gentle iron and Omega 3.
Mentals during menses are mainly desperation! She's fearful for her health - but since she's losing so much blood every month I don't know if I can use that to determine the remedy - if she wasn't worried that would definitely be RSP.
Any ideas on when to start the remedy in this case? I'm not sure whether to give the remedy before menses or only on onset of heavy flow. In addition - if heavy flow is stopped in mid-process, does that mean that the body is not "cleansing" itself sufficiently? Or is this kind of flow more of a haemorrhage and menses is just an opportunity for this? Not sure this is clearly expressed - hope someone understands!

Re -heavy flow I often find that a dose or 2 of Sabina 30 before the menses or immediately it starts can stop it being so heavy. Sometimes the menses will go on a couple of days longer because of this.

Perhaps this has nothing to do with the "menses", i.e. the cycle
itself, but with the additional loss of blood. Did you say that it was
all through her menses, i. e. also at the beginning, when the
additional loss of blood has not occurred yet?
The heavy loss of blood is also IMO not connected with the cycle
itself but is caused by the fibroid. This is common - although not
usually to that extent.
I should rep. for rubrics like "small wounds bleed much" etc. to cover
the latter aspect.

One suggestion -- only addressing one of your questions: Since this is a chronic state, you could start the remedy whenever you've decided which one, I would think. Obviously the menses and what happens would in a while give you an indication (possibly worse before better) of the remedy's action, but often there's an immediate shift in the energetic realm that would give you another important clue.

Hi all, thanks for input.
The urinary symptom is part of the PMS picture. Within the PMS picture there is also extreme tiredness and irritability - before the flow starts. Oddly enough before this picture emerged, when she was younger, the PMS included some irritability but also very high energy and focus, it was a good time to get things done. She told me her mother was like this, and always used to schedule driving tests etc. for the days just before her period.
