Siblings didn't menstruate until age 14 so I don't think puberty is relevant
to the case.

Background. This 11 yr old girl, presented as a slim tall child with long
brown hair, brown eyes and was dressed in jeans and a black jumper wit a
yellow shirt on top. She smiled a lot and had quite sparkly eyes. She was
very open to me and answered my questions the best she could. She had a
febrile convulsion at 18 months and has had chicken pox. She has had all her
vaccinations with no problems. She currently uses 1% hydrocortisone cream.
While she was with me she drew a picture of a dog and wrote on it - not a
scary one!
Presenting complaint - eczema
Eczema - looked red dry with horizontal lines, bits fall off which are
yellow. It is itchy but she can ignore it. The itch is < day as if doesn't
stop her sleeping. It presents in the inside of her left arm and on both top
and bottom lids of her eyes. This eczema started 2 weeks ago. She has had
none for 18 months. There appears to be no causation. It is possibly worse
in hot water in a bath but she now tends to have a shower which doesn't
affect it as much.
At 18 months she had eczema on her ankles for 12+ months and after that it
has come and gone.
She had a headache a on a recent hot day when it was the school sports day
and she was dehydrated.
In the summer she can get dry skin under her big toe which may crack.
When she was little she got cramps in her legs in the calf muscles.
When at school she can be "shy and gloomy" although she appeared giggly and
smiled a lot to me. "I am shy because I don't want other people to think I
am weird or strange." Weird? "Means crazy, strange, unusual, and
mysterious." Unusual "I like reading and most kids don't. I wear different
clothes than others. I read fantasy and I like dark clothes with something
bright."
Mum says that at home she is bubbly and doesn't like attention at school. At
home she mimics people. She is intelligent.
Anger - "not very often". When she is "I stomp off on my own". She couldn't
recall the last time.
Unhappy - "When I get worried. I am worried about moving to secondary school
next term. I am worried about getting lost there and forgetting stuff to
take with me."
Cry- If she cries she wants to be on her own and if anyone came up to her
she would tell them to "go away".
Mum says that she always washes her hands after going to the toilet even if
she is about to get in the bath, however on talking to her this appears to
be habit rather than compulsion or fear of germs.
Fears - Spiders**
- heights
- Little snappy dogs.
A couple of weeks ago she had a vomiting and diarrhoea acute for a couple of
day which wasn't treated.
Food - desires sweet. Aversion - spicy, cauliflower, tropical fruits like
mango and pineapple. Likes fried eggs and dipping the yolk in toast but not
boiled.
Drinks - fruit juices and milk but not everyday.
Pregnancy- trouble free. Birth - quick and easy. She was breastfed for 9
months.
Teeth - 6 months
Walked - 12 months
Talked - average
She has 2 sisters 14 and 17.
Had a febrile convulsion once with a fever and a couple of ear infections in
her life but other than that has been a healthy chid.
Environment - she likes the town and prefers cold weather because she can
wrap up. She doesn't like the rain and doesn't like the heat. She feels the
heat more than the cold.
Company - She would rather be with a small group of people.
Sweat - mum said that she started to have a body odour under her arms from
the age of about 9 years old and has used deodorant since then. She looks to
be no where near puberty at the moment.
She is tidy
"If the whole class is being punished when I have not done anything wrong I
get annoyed". She doesn't say anything though.
Sensitive to mum's feelings. She wouldn't want to hurt others as it would
make her feel guilty.
Sleep - good Dreams - weird "When I walked down the stairs I took my shoes
off and this old lady put them on. When she took them off and I out them on
again they were too big. Another was someone making sweets from broccoli and
that tasted nice."
Teeth - she has no fillings but grinds her teeth.
She is not very coordinated with sports like tennis or rounders.
Information from teacher - in school very withdrawn, very level headed and
serious. In the past has cried when she got 9/10 rather than full marks.
Doesn't fit in. She has 1 friend and they are very close. Spends lots of
time with her family who coluld be called geeky. When younger she used to
blush if anyone looked at her.
Family Medical History
Thyroid problems - maternal grandmother has had hers removed
Mother - enlarged thyroid during pregnancy with her sister who had an over
active thyroid from age 13 and is on medication for it.
