This is one of those cases where a full case-taking and a regular follow-up schedule has not been possible, but I know the patient well and he asked for help.
The dental work has been extensive and is due to carry on for a period of about 2 years. He's having his entire mouth "renovated". So it's not one tooth or another. He has been doing this for one year approx so far, and has not had trouble with bleeding during dental treatment. The only time this happened is when he took Arnica just before dental treatment. So it would seem that the Arnica taken prior to the treatment caused the bleeding, especially since this is such a strong symptom of the remedy. I think some kind of proving was taking place. He has been taking 30C at this time, although I gave him 30C and 200C in the original acute. I will look into the possibility of complementary remedies.
Something about the case: The patient called me the day after doing some strenuous work, saying he had terrific pain in his foot, which started some time during the night and was getting worse. It turns out that the evening before, while he was working, he had had some trauma to the foot but ignored it so completely that it was hard for him to remember that something may have happened. (here is where the Arnica saying "I'm well" comes in). I would have suggested Arnica anyway for the trauma and response to trauma, but it seemed even more strongly present because of the way he continued to work. Arnica 30 started helping the situation and 200 completed the work.
The way the patient described the "sense of well-being" was very similar to the way other patients have sometimes described a first response to a remedy that has really helped them - a feeling that "this is good", sometimes even a sense of comfort mingled with awe even though that sounds strange. It's not the same (response to Joy) as when the patient says he feels good when he's covering up deep sickness. It's also not long lasting, just a preliminary meeting with the remedy. Patients ask if they can repeat the remedy just to get that feeling back again - but it's the kind of euphoria that it's healthy to experience from time to time but probably not the best place to live... Has anyone else experienced this with remedies?
On reflection regarding the nature of the patient, much of chronic Arnica's pushing people away when needing them, closing off and unfriendliness found expression. His response to emotional difficulties is also to close off, shut down. So the next time he called with knee pain, I thought why not, and gave the Arnica again with good results. The original acute he had also seemed very connected to a chronic situation where a patient ignores the body's cries for help and forces himself to continue working and functioning.
On the one hand I know there is the view that the constitutional cures all - but there is also the method of working with an acute intercurrent. (I usually give the constitutional first in acute, unless strong general modalities have changed. Sometimes it helps, sometimes it doesn't) The dental treatment definitely counts as an external trauma and the idea of relating to Arnica as the chronic and looking for the best intercurrent acute seems like a good idea. My feeling is to go with Staphysagria, both because of the knife wound element in dental treatment, the constant cutting he's going through, and also because it connects to personality traits as well.
Any other opinions and input are very welcome!

Alcohol is supposed to be inimical to Arnica ( wine) and increase the
unpleasant effects - anything like that happening?
Ipecac is complementary to Arnica, follows it well and also is an
antidote and Calendula also follows Arnica well. Calendula has
hemostatic properties after tooth extraction as well. See if there is
any fit for the patient in the acute bleeding phase in either of these
two.
If Arnica looks like his constitutional then one of its related
medicines or antidote should work in the acute phase.... ( Would you
give a person's constitutional just before an acute procedure?
Of course in this case he is undergoing a procedure for which Arnica
might be the acute medicine as well.)
Considering all of this rearchitecture work that has been going on
inside his mouth - Staph, Hekla, Hypericum etc. might also be indicated.
Probably probe a little more into what are the actual teeth involved,
pus, pain, modalities, other concomitants and you could get a
follow-up remedy.

is a man in his late 50s, who received Arnica after trauma to his foot
about half a year ago. The minute he took the remedy, even before the
pain relief, he said he felt a tremendous sense of well being in his
entire body (and mind). The arnica was very helpful to him. Because
of his reaction, the next time he felt unwell I gave him arnica again,
even though it wasn't an acute, and again the response was very good.
started, I gave him arnica, which helped so much he decided on his own
to take it BEFORE the next procedure. Unfortunately, during the next
procedure he started bleeding heavily and the dentists couldn't
continue - which teaches a lot about arnica prophylaxis as a
double-edged sword.
dental procedure where there is a while to go before the next one, he
can use Arnica, but if procedures are close to each other he may land
up with bleeding again. He's just had dental implants put in and is
in some pain, but is due for a root canal in 3 weeks time. I'm
thinking of telling him to take the arnica but only over 2 or 3 days
at most, to make sure that the remedy won't interfere with the root
canal. What do you think? Arnica is clearly a chronic for him, and
many of the mentals, generals and physicals agree.
