If there is a return of an old symptom but with completely different
modalities....is it really an old symptom or is it a new symptom? As in a
migraine that starts at a completely different time and on the opposite side
of the head.

If the changes are that distinct, I would think likely a new symptom.
Is this something that happened after a dose(s) of remedy?
Also, return of an old symptom -- as we generally talk about it -- would happen in context of an overall health which has improved. Is that the case here?

IMO, a symptom or sign is never fully expressed without its modalities (and concomitants), therefore if any part of it changes, yes it is a new one.
You give the example of migraine, which in itself is a very vague term that needs definition and explanation by the patient; but let's say the complaint is pulsating headache.
The original complaint/symptom could be related to food sensitivity for example; OK, so the offending food has been removed; now another pulsating headache comes up...the modalities being different, the remedy will probably be different, even if you deal only with a symptomatic/palliative treatment. The real origin of the symptom/complaint might need a completely different approach....but you know that very well.
In short, an "old" symptom with different modalities is not a return of old symptoms but the emergence of a new presentation on the background of a sensitised organ/system that will be the first to react.
Does that make sense?

I knew they were related to the migraines in some way because I would get a metallic taste in my mouth with the cramp.
*******Zinc has side effects of metalic taste in mouth and abdominal pain, especially on an empty stomach.)

Will Taylor puts and more important Boenninghausen valued modalities way above location (your head for your headache). So, a change in modalities would ask for a change of remedies. Very smart question.

Thanks for the insightful/helpful answers. Homeopath still won't tell me the
remedy but I asked the potency and it was the first time that the homeopath
had given me a 200c. I must tell you I've been suffering a lot in the last
month. The fact that there was a return of a two year old symptom is I suppose
a good sign but I'd rather die than go through that again. I was asked about
my well being. I'd say that the last month I've spent an inordinate amount of
time reading up on stuff on the internet about the history of my
ancestors(Scots-Irish) and so have spent a great deal of time in my head. I
have also thought just about every day that I'm having a heart attack and
guessed that she must have given me Kali-ars. I haven't gotten angry or
frustrated by my husband and that is very unusual. I think I would feel better
if I could yell at him now and then. I have told her about the Fibonacci
series and next time I'm going to insist on it. I have no idea if I'm coming
or going. It has been over a month and she is not anxious to find another
remedy...wants to wait and see. I also feel like I look ten years old when I
look in the mirror. 200c must be a major physical detoxer and I feel it might
lead to my early death.

Modalities are the symptoms of the patient not symptoms of the disease itself. If you have changed the modalities would change.
