Hi,

Dr. Shahrdar's Free course on "Case Analysis when there is a Paucity
of Symptoms" is in full swing. Currently we have 31 members.

I have posted the analysis to Case 2 below. Below the case, you will
see Dr. Shahrdar's analysis of the homework. To view the

Case 2

• The patient is a 29 year old woman with the complaint of
sleeplessness. The problem began with a deep emotional relationship
that broke up about 3 months ago. As she was steeped in sadness, so a
psychiatrist prescribed Citalopram (Citalex). After taking Citalopram,
which she still continues, she feels better with regard to the
sadness, but still has problem with sleeplessness. Sleeping pills do
not help her. Sleeplessness is accompanied by palpitation. She tosses
about in bed troubled by disturbing thoughts. One of the main
problems, that is also related to her sleeplessness, is
oversensitivity to noise which was present from the beginning of her
story.

• She wakes up from the slight noise. Everyone in the house must be
cautious not to make the slightest noise. Recently for the last 10
days, she is experiencing photophobia and has had 5 episodes of
epistaxis that she had never experienced before. Photophobia is
present during day. Epistaxis is with bright blood bleeding after no
apparent significant trigger. During epistaxis, she feels dizzy and
reports an increased photophobia during it.

Case analysis

• What is the central theme of the case being the target of remedy prescription?

• What kind of approach for case analysis do you follow?

• How do you evaluate the symptoms? You should also look up the side
affects of all medications.

• Translate the signs and symptoms to rubrics and repertorize them.

Classwork

• This is also an NBWS case, but there is a big difference between
this case and case no. 1. What happened at the beginning of the
process was a psychological event. When an altered condition (a
pathological condition?) follows psychological crisis, two things may
happen. What follows is either a chronic non-miasmatic psychological
state or a flare-up of a latent miasmatic state. When you see pure
psychological symptoms with no signs of physical miasmatic features,
it is most probably a non-miasmatic psychological state. What about
this case? Presence of sleeplessness, restlessness, and
oversensitivity, suggests a chronic non-miasmatic psychological state.
But what about epistaxis and photophobia?

• Both of the above physical symptoms may point to presence of a
miasm. This means that we maybe we are witnessing a miasmatic
condition. BUT.... She is taking Citalopram. If you look at the side
effects of Citalopram, you will find epistaxis and photophobia as
important side effects. This means that these symptoms are actually
the

'provings' of Citalopram. In analyzing chronic conditions, if a
patient is under the effect of a medicinal disease, you should not
take the symptoms caused by medication in the case analysis. So by
using a filter that only chooses dynamic symptoms, what is left is a
pure psychological state. So in this case, there is no need to refer
to a miasmatic Genus Epidemicus.

• The best remedy among the high rank remedies in the result of
repertorization is Coffea cruda. Referring to its materia medica, it
is one of the best remedies for hypersensitivity and sleeplessness
following both good and bad emotional excitements. Although Ignatia
amara is the first remedy in the list, I prefer to choose Coff as its
efficacy in treatment of type of sleep disorder that this case
exemplifies is much more prominent than Ignatia.
Take-home Lesson

• Always check the side effects of your patient's medications.
Reactive chronic patterns cannot be removed by homeopathic remedies
and the symptoms interfere with the symptoms and dynamic state and
lead to an ineffective repertorization.

• Do not think that you need a miasmatic approach in every case.

• There are many remedies that may be indicated in 'Ailments after
disappointed love'. Do not just think of remedies like Ignatia and
Nat-m.
