Dear All, here is a case from Rhonda who would like some help with it.

His symptoms involved lots of discomfort at the base of his spine, slipped
discs, shooting pains up to the back of his neck, causing difficult
concentration. Pt is a script writer, and spends lots of
S/s began 4 years. Noticed gradual onset. Was spending a lot of time at
computor (writing scripts). Changed chairs, began running a lot,
basketball.
Began seeing chiropractor. Noticed constant "annoyance and discomfort" at
base of spine. Couldn't lift arms above head without difficulty. Began
specific corrective exercises from chiropractor. > cold applications.
Weakness in mid thoracic area. Grinding sensation in this area. Pains
shooting up to back of neck, making concentration difficult. Joints,
especially knees click(both sides).
Pattern since then: ligaments not holding adjustments in place(laxity
problem??). Slipped disc. Pelvis weak. Described pain on 2nd visit as"
pressing, constant, uncomfortable... I feel miserable, frustrated...,"

Currently: lifting is not a problem, has full range of movement, can lift
arms above his head without difficulty. Adjustments are holding in place
longer, back seems stronger. Experiences "muscular pain, rather than in the
bones". By patient's admission:"I'm better when not thinking about my
back".

Dear Rhonda,

I would like to make a few comments regarding your case:-

If you don't mind me saying I am a little concerned that he is taking more
than one remedy at a time and for such a long time + there are supplements
involved which I don't have essential authority to comment on but I notice
that Zinc is involved as well. Apart from the fact that his rectum sx don't
seem to have been improved it is very difficult to know what remedy is doing
what here.

Anyway I began with these rubrics from Kent - rectum, excoriation from acrid
moisture and also moisture after stool and looking at the remedies in these
rubrics I notice that CARBO VEG is there in the first, which interested me
the most.

So, from Clarke CARBO VEG =

frequent stools
liquid evacuations
corrosive discharge from rectum
much flatulence
obstruction of nose (loss of smell due to?)
slow thinking, << afternoon generally + sudden and periodical weakness of
memory (both these sx would cover the 2-4pm aggravation)

timid and embarrassed in society (shyness)

lots of yellow/brownish expectoration
shooting pains in back
pressing pain in lumber and sacral region of back (Kent)
rigidity of the spine
painful stiffness of back especially when rising in the morning
pain as from dislocation
sufferings as from a strain of the back
great debility as soon as any effort is made
the skin has lymphatic swellings (not sure this would cover the lipomas)
lots of moles on the skin (kent)

Also when I am presented with someone who has back problems I always think
they are carrying burdens, a weight that hurts and goes to the back and I am
wondering if he has told you all you really need to know about why he has
these back problems and also why he has the 'leakage' from the rectum.
Something is oozing out, trying to come out - maybe he hasn't told you yet.

I note also that you have, at some point, prescribed Lycopodium. This is
interesting in as much that there is a connection with Lyc and Carbo veg -
many of the MM's say that to avoid the aggravations that Lyc can cause one
should give Carbo Veg first.

More importantly you say he has had a skin cancer removed from his back -
this really needs to be addressed - what is going on with this, it really
makes the warning signs flash to me.

At least for the time being I would suggest looking at Carbo Veg but also
dig a bit deeper as to causation and take a closer look at the skin sx.

One last thing - a question. Has he ever had the small pox vaccination prior
to his sx developing?

I, like Joy, came to Carb-v

yielding (does what you ask)
timid
acrid discharge from rectum
agg from shellfish, mussels (not exactly squid, but close)

I did not even consider the flatulence which is of course is covered.

This being said, I agree with Joy on looking at causation 18years ago,
looking at generals (thermal reactions, thirst, etc) before making a final
choice.

I am also concerned about all the modalities involved and trying to unravel
the case. In my book, the patient appears suppressed. Muscle soreness/pains
are deeper symptoms than not holding adjustments. I would like to see the
bowel function improve earlier than the muscle and back sx, too.

Thanks for the comments on this case. I have been attempting ever since the
first visit to establish what possible causation(i.e. 18 years ago) may
exist. My thoughts are (1)the patient genuinely does not remember,(2) does
not want to tell me. Has anyone had this experience where information is
not "available". I've just been trying to work with what I have. As I said
in my comments in the case, I feel patient has come a long way with his back
problems(and my experience with nutritional supplements in separate patients
indicates that they work for strengthening connective tissue etc). He is
currently away on work, but when he gets back I'll ask re smallpox
vaccination, and test his memory bank once more for past symptoms. I truly
think he can't recall, as he is open enough with me to talk about family
history regarding venereal diseases, and s/s(signs and symptoms). Carb-veg
seems a good match for his bowel symptoms and wind - will let you know the
results.
