Encapsulated TB
In Kent (?) somewhere there is reference to caution with a remedy in case it can break open an encapsulated TB lesion in the lung. Does anyone remember what remedy this is.

Sil. & Phos. would be at the top of the list of those to avoid or use with EXTREME caution;
I'd also think that Tub. might be in the short -list.
Basically the idea here is that the VF could be incited into "too much cure" .. which could result in suppurating of tubercules & reactivation of dormant TB .. we can use that concept as a caution, regardless of which remedy is being considered..
-then, following on that concept, we add in "the usual"
pace and depth of pathology
pace of Rx
sensitivity of client
-and evaluate these together, to come to posology
Note: since Kent was not aware of liquid posology, we have a potentially huge advantage there .. further, with LM in liquid posology, we have the potential for starting out with very minimal dose, and the ability to repeat (judiciously) so as to "speed the cure" as Hahnemann stated (aphorism 246 of Organon 5th & 6th editions) "in 1/2, 1/4 or less" the time taken with old methods of 4th Organon.

Phosphorus is a dangerous medicine to give very high in some cases of phthisis, in the last stages of phthisis. In this case they should have received Phosphorus when they were yet curable. In these cases Phosphorus 30 th may sometimes be used with safety and it will act as a test in doubtful cases to see whether reaction can be brought about. In such cases where reaction can be brought about the administration later of a still higher potency may be found useful, but in the beginning withPhosphorus in phthisical cases far advanced it is better not to go higher than the 30 th or 200 th.

Thanks for that and I am sure I remember far back being told about Phosphorus. I think it came up in a case we did at college.
The case today is a sweet woman, age 79 (previously a homeopath)with dementia and hyperthyroid +goitre and is rapidly losing weight. I am using Bar Carb - a 200 improves things but doesn't last so I have gone up in potency. She is very lucid sometimes but tells me she doesn't know who she is or where she is sometimes. A MRI showed that there is scar tissue in her brain from a mild stroke years ago which may be pressing on the short term memory parts of her brain tissue. She had TB in her youth.

Without knowing any more than you've mentioned, a couple of things come to mind:
First, are you aware of and using the Rx as liquid posology
(some people say "water potency" I think?)
Second, are you using LM at all ?
The reason for this line of questioning .. is your mention of 200c "not holding"
-this is exactly the type of chronic case which is most suited to treatment by LM potency scale.
Since you say you've had good result with Bar.C, you could continue on with it ad-infinitum, going up the LM scale as needed.
For practitioners who aren't aware of LM scale, (the "Kentian" perhaps?) your case might be coming to a spot where an intercurrent multimiasmic such as Sulph. or Tub. (which are "follows well" in relationship with Bar.C) would be useful in the case.
I would think that caution (as mentioned in previous email) would be recommended in Sulph in this case as well as Tub.

I am fully aware of both LM prescribing and Water Potency a la Luc de Schepper. I am one of those people that doesn't seem to get on well with LM's re patient compliance but I do frequently use WPs . With this particular woman even that would not work as I would be reliant on someone else administrating it to her. Her daughter who brings her to me and is also a patient keeps losing any daily remedies I wish to give her. Sometimes I wonder why?????
Sulphur would not be an option for this very chilly woman and apparently she has been given Tub by her last homeopath.

have used iodum 30c 1 drop daily water dose in similar cases with success. Of course backup nutritional supplements etc such as uncooked rehmannia and alfalfa can be helpful
If the patient is rapidly losing weight medical intervention may be warranted

She has an appointment on May 17th with a consultant after blood tests showed the hyperthyroid condition and in the meanwhile is taking Indural to protect from a heart attack.

you no doubt have her on other supplements such as high dose magnesium, epa/dha, Co-Q10 to protect her heart
Small meals often with high protein would also be something to consider suggesting
