I'd like to thank everyone who offered ideas and suggestions. He seems to be doing better on the Chamomilla. His weight has gone up about 2 ounces. That isn't much, but it's above the weight he's been for the last 2 months, so any gain is good.

A quick update since my son started Chamomilla.
He gained about 2 ounces right away. Then I gave him the remedy again in the village Pacaran. He gained a whole pound over the weekend. This is after not gaining any weight for the last 2 months.
This also confirms my hypothesis that remedies works better in Pacaran. When we went to Pacaran without the remedy, he did not gain any weight. (So it isn't the place alone.) When we used the remedy here, he gained some weight but not much. When we used the remedy in Pacaran, he made a dramatic improvement.

So for him, clearly (or at least it *seems* clear) EMR is an "obstacle
to cure".
I'm eager to hear what happens as things move along! I wonder whether
*eventually* he can become able to tolerate EMR? I would not assume
either way, but do hope you'll update us periodically as things unfold.
