Repeating words under breath?

I have a little guy (5) who has started repeating back words under his
breath. So he will say something and then he will say the same thing
again under his breath (like just mouthing the words). He says he is
not aware he is doing this.

He also has started undressing completely to have a BM (think Kastanza
from Seinfeld).

He also seems slightly concerned with germs (like eating from someone
else's food) but not fastidious and no excessive handwashing.

Any suggestions are most welcome. I can't seem to find a rubric for
either compulsion.

a few ideas

mind, repeats, the same action

speech, cannot finish sentence (a polarity of what you wrote but could be significant if the child has a mental confusion of some kind such as a learning difficulty)

speech, forgetful of words while speaking (same as above - the need to repeat is a way of letting the words sink in)

mind, speech, repeats the same thing

mind, talks to self

mind, imitates, mimicry

uncovering >> (although it reads as though there might be a connection with undressing before going to the toilet and the interest in germs, as though he doesn't want to soil his clothes)

mind, disgust with dirt

fastidious, for cleanliness

an evolving Hyoscyamus state?

Yes, *very* helpful. Hmmm...I'll have to think about that as I don't
see as much Hyos as Stramonium. Overall, he's a very gentle, polite
boy. But does have outbursts. I thought Ferrum for a bit - obsessed
with swords and guns right now but not atypical for that age and
with Pirates of Carribean, etc.

He's extremely modest for a 5 year old. Very easily embarrassed.
I'll have to look at both some more. Main complaint is intermittent
high fevers which are usually on Wednesdays but sometimes Fridays or
Mondays. It's very involved. The compulsions are new symptoms.
