any ideas on this one? seems to be connected to anxiety. The vomiting is explosive and loud.

Sounds like what we have just been through all last night with our
youngest - some gastro bug going around. Anyway there is vomiting
forcible + vomiting on waking + vomiting with anxiety + anxiety on
vomiting + choking after vomiting, although choking is so easy when
vomiting and so is being anxious when vomiting. Ratanhia and Sambucus
have something like you describe but so has Arsenicum and possibly
Ipecac and some of the snakes and heart rx. If it is the anxiety that
is causing the vomiting then use the anticipation and anxiety rubrics

...does the choking/vomiting only happen at night? Is there any
accompanying cough? Are there any other related Sxs? Is this a young
or older Pt? Does the vomit happen first then the choking..or the
other way around? Any specific time of night?

thanks for this and other replies. I don't think it's sleep apnea, it seems to be very much related to anxiety about finances, and also a sense of inadequacy in the marriage, as he feels his wife should be helping or sympathising and she doesn't. It's only at night, but I didn't get a specific time. he wakes in a panic, feels he's choking, and then goes to explosively vomit.
there's no cough, but reflux is an interesting idea.

One additional possibility to consider. The panic, choking and vomit are common symptoms with hiatal hernia. It usually occurs in a phase of very deep sleep when the nerves and muscles relax to the point a small amount of stomach acid starts up the esophagus. That burn in the throat starts a violent cough and/or choking sensation which can go into spasmodic form of vomiting. Very uncomfortable. I have several females with this problem - which started when pregnant with large babies. In their cases, anxiety plays a part in their sleep cycle - and the time at which they finally go into the deep sleep phase. Both 'reflux' (which appears to be a combination of diet, stomach chemistry (pH), and nerve related issues) and the hiatal hernia should be medically confirmed since there are other problems which should be ruled out.
