Is nux-m at all relevant to your case? The description of needing someone else to tell her to do simple things, the migraines, and the sense of not being there made me think in this direction. Also, the disassociation leads me to think in terms of a drug remedy.

The centrality of headaches makes me wonder about Kali-br or Thuja, but there's nothing that really catches the imagination among the rubrics or core symptoms for those remedies, outside the headaches, and the headaches don't really fit, as far as I can tell with so little to go on.
One might think of "absence," as well, but I understand that rubric as involving total absence, as in petit mal seizures, and she hasn't had anything identifiable along those lines -- just migraines and muscle tension headaches.
Obviously it's some sort of dissociation, perhaps even to the extent of catatonia, but again, not quite -- with outside direction she's perfectly capable of doing whatever she's told -- she just can't think independently to do what she wants. The case is long and complex, and I'm just wondering if anyone has a rubric thought.
