This is a difficult case, in which homeoapthic treatment is not helping the patient, because the right remedy is not found.
The case was repertorized many times, but the remedies (always polychrests) - never helped him.
Untill 3 years ago he was very much helped by Selenium 30c (for about 6 months). From then on - no progress achieved.
I am missing the TRUE picture . Therefore, I am posting the case to ask you for your own view and opinion.
Please, I am not asking anyone to repertorize the full sympom picture of the case. Just look at it and say what is your impression, what jumps first to your mind when you read the symptoms.
The patient is a male 45 years old. He is pale, thin, anaemic, chilly, has low blood pressure, most of his day and night he spends in because standing or walking or sitting tire him and also because LYING amelliorates some of his symptoms (like weakness, sinking stomach after stool and more).
His main problems in short : (for the last 7 years) he is weak (chronic fatigue), has "brain fag", tired eyes and chronic gastritis and very much aggravated by an afternoon sleep (even a short sleep) . When he is weak in the morning - the only thing that causes the weakness to disapper is even instantly is: stool (hence: MIND - CHEERFUL - stool - after).

Unfortunately, your list of rubrics makes it extremely difficult to create a mental image of what symptom(s) might be SRPs in this case, or MUST be included in consideration of the remedy. What is your sense of this? In your opinion, what is/are the most striking features and the most central ones to this patient? That's where the focus must be in finding the remedy. Often if you have a smallish (under 20 rx) rubric of which you're very sure, and you scan the remedies in that rubric with this patient in mind, one or two of those remedies will be distinct possibilities for this patient that you hadn't previously considered. FWIW...

I don't think one can get much impression from a list of rubrics, because
there is no way to evaluate which are more important, or severe, or common
to situations, or etc. Also it could be useful to know how certain symptoms
were given -- offered versus asked for; expressed easily or with difficulty,
expressions, etc. I know that giving a shortish write-up of a troublesome
case is no picnic, but I *think* it might offer more chance of getting
useful comments?
Maybe it would also help your thinking process to tell us more about the
person, what you feel are the more important aspects to prescribe on, things
like that? Maybe there are aspects of the case, or casetaking, or your
relationship to the case, or whatever, that talking should help to "shake
loose"? (I'm thinking about how I can get mentally gridlocked by a
frustrating case!)

Phosphorus seems an obvious choice even though <<< short sleep (Phos has
this as well, they feel as is they haven't slept enough and this causes the
aggravation).
Have any of the kaliums or acids been tried?

Selenium seems to be a good choice. Did you try increasing the
potency?
Borax has cheerfulness after stool.It has gastro intestinal
complaints as well. Perhaps Alumina, Lyco, Zinc,Caladium, Nat. Sulph
etc. could be considered.
